AdaptAbilities HIA Summer Camp Registration Form

Note:

• Due to program popularity, your child may be placed on a waitlist upon registration.

Registration Requirements
The following is required to complete registration:

1. Create Account on Family Portal
2. Consent Forms and Participant Profile are up to date on Family Portal
3. Current headshot (chest and up) photo uploaded on Family Portal
4. $100 deposit payable by credit required for online form; please complete debit or cash payments at Head Office
5. Copy of Funding Agreement on file (FSCD, PDD, Children's Services etc.)
6. Pre-authorized payment form on file. Forms available here.

Camp Confirmation

We will finalize camp details based on registration numbers and health restrictions. Registration is not confirmed until an email is received by the HIA Summer Camp Team which details if a spot is confirmed, or if you have been placed on a waitlist.

Parental Cost Shares

• Parental cost shares for registered weeks paid in full prior to the first day of service
• $100 deposit will be applied against parental cost shares
• $90 Weekly operational fee for camps that are funded on an hourly basis

Cancellations

• Due to COVID-19 restrictions or low registration, we reserve the right to cancel certain camps
• Deposits are refundable for COVID- related issues
• Refer to our Parent Handbook for our Cancellation Policy for all other circumstances

Participants that have not previously attended programming at AdaptAbilities must complete an intake prior to completing this registration form.

Funding Confirmation

Families are required to contact their case worker to request Specialized Camp funding.

Camp & Respite Hours

If you require cancellations, please contact registration@adaptabilities.ca . Before and after care are available for an additional fee. If you are adding hours to before/ after care, AdaptAbilities makes every effort to accommodate. We require a minimum 14 days notice prior to changing hours to requested care to avoid hourly billing.

T-Shirt

T-shirts are for participants to keep and wear during camp. Order include one free T-shirt. Additional T-shirts can be purchased at $10 each. Please select the appropriate sized T-shirt for participant.

Camp Selection

HIA Summer Camps provides enjoyable, goal-directed activities during the summer. Small group settings foster the growth of the individual- socially, physically, mentally and emotionally. Teens and Adults explore city festivals, community and recreational events.

Locations are not guaranteed and location availability is pending sufficient registration. Camp locations will be confirmed at least one month in advance.

Camp Deposit

AdaptAbilities requires a Camp Deposit to secure your space in programming. The Camp Deposit is $100 and will be applied towards your Parent Portion.

For Families we are unable to accommodate, AdaptAbilities will refund the Camp Deposit.

Two ways to pay:

- Online via AdaptAbilities Website (Credit) 
- In person at Head Office only. Head Office is located at 10130 82 Avenue, Edmonton. Office Hours are 8:30 AM - 4:30 PM, Monday through Friday. 

Registrations will not be accepted without payment of deposit at time of submission.
